Text For Citation: 01 Item/Group: 001 Hazard: ELECTRIC
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to slips/trips and stepping into floor holes: a) On or about June 6, 2005 and at times prior Timpas Feedyard LLC did not ensure that employees were protected from slips/trips and holes in walking/working surfaces. 1. Grates over the grain dump areas were missing pipes leaving openings that could be stepped through. 2. Grain was allowed to build up in piles by augers causing slippery areas to walk through. 3. Holes in the floor of the cooking area were improperly covered or not covered at all. 4. Holes in the scale table were improperly covered or not covered at all. 5. Holes and poorly supported metal decking on the roof near the auger exposed the employees to trip hazards. Among other methods, one feasible and acceptable method of abatement to correct this hazard is: a) Install covers over the holes in a proper manner, evaluate and repair the roof area where the employees have to walk, and develop and maintain a house keeping plan for the areas near augers. Abatement Note: Abatement certification and documentation is required for this item (see enclosed "Sample Abatement-Certification Letter").
