29 CFR 1910.252(a)(2)(iv): The individual responsible for authorizing cutting or welding operations did not designate precautions to be followed in granting authorization to proceed preferably in the form of a written permit. On or about November 7, 2015, employees engaged in the use of oxygen and acetylene cutting torch operations were exposed to explosion hazards due to the employer not providing a hot work permit prior to beginning work.
