29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, or uprights did not bear on base plates, mud sills or other adequate firm foundation: On or about October 28, 2015, at the above addressed job site, employees were exposed to a fall of approximately 21 feet and 9 inches above a lower level while removing forms from a scaffold built with base plates on top of insufficient amount of mud sills.
