Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of explosive release of hot caustic material and to flying debris: a.At the establishment, wall thickness determinations were not made on digester shell walls prior to making weld overlay repairs on batch digesters 5, 8, 9, 11 through 15, 17, 19, 20, and 21. Among other methods, one feasible and acceptable abatement method to correct this hazard is to implement an in-service inspection of pressure vessel program as required by National Board Inspection Code (NBICANSI/NB-23), Chapter V, and/or the requirement for wall thickness determination as required by Stone Container Corporation Welding Overlay and Repair Procedure RP134, Batch Digester Weld Overlays and Repairs, Section 6.0. b.At the establishment, progressive inspection and maintenance records were not maintained for the locations and the thicknesses of critical inspection and repair areas such as, but not limited to through-shell repairs, overlay repairs and thickness measurements of overlay repair areas on batch digesters 1 through 22. Among other methods, one feasible and acceptable abatement method to correct this hazard is to implement a recordkeeping program in accordance with the provisions outlined in National Board Inspection Code (NBICANSI/NB-23), Chapter V, Paragraph U-104 and/or recordkeeping requirements listed in Stone Container Corporation Welding Overlay and Repair Procedure RP134, Batch Digester Weld Overlays and Repairs. c.At the establishment, repair stamping and nameplate attachment required by National Board Inspection Code (NBIC-ANSI/NB-23), Chapter III, Paragraph R-403, were not performed on digesters, including but not limited to batch digesters 15 and 9. Among other methods, one feasible and acceptable abatement method to correct this hazard is to implement a repair and alteration program in accordance with NBIC, Chapter III, Paragraph 403.graph
