Text For Citation: 01 Item/Group: 001 Hazard:
29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a. At the site, employees were exposed to struck by hazards, in that, training and education was not provided relating to safety requirements for excavations, to include at a minimum, shoring and sloping requirements, the various types of soil classifications, placing and handling the spoil so that it did not present a hazard to employees, safe means of egress to and from an excavation, on or about 10/21/2015. b. At the site, employees were exposed to a struck-by hazard, in that, training and education was not provided relating to hazards such as, but not limited to, handling material while inside a trench by using the excavator bucket to lower material such as gravel over the employees, on or about 10/21/2015.
