29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: For Meadow Burke establishment at 511 Santa Fe Road, Tampa, FL 33619, as observed on or about 10/15/2015, employees were exposed to amputation hazards as a result of operation of the following energized machinery: a) A Meadow Burke Beam Bolster was being locked out tagged out with the Emergency Stop Button being enclosed as a lockout tag out method, employee suffered right finger tip amputation when trying to clear a jam on or about 10/06/2015 while machine was still energized.
