Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: S/C the hazard of having part of their body caught between the moving elevator car and the hoistway landings. One feasible and acceptable method, among others to correct this hazard is to provide interlocks for the elevator car gate and the landing doors to conform with ANSI/ASME A17.3-1986 part II, sec. 2.7 as referred by part IV, sec. 4.1 a) 568 Broadway, Crosby Street side - sub-basement to 12th floor - Elevators #1, 2, 5, and 6 were not provided with interlocks to prevent the operators from running the elevators with car gates and/or landing doors open. On or about 2/6/91.
