Text For Citation: 01 Item/Group: 001 Hazard: FALLING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the collapse of aerial lifts: WORKSITE: ON OR ABOUT SEPTEMBER 14, 1998 THE TWO 6 X 37 CONSTRUCTED WIRE ROPE (DRIVE CABLES) FAILED WHICH CAUSED THE UPPER AND LOWER BOOM ON THE MODEL LR 50, SERIAL NUMBER 73-3033, ASPLUNDH AERIAL LIFT TO COLLAPSE. THE EMPLOYER FAILED TO MAINTAIN AND INSPECT THE AERIAL LIFT'S DRIVE CABLES IN ACCORDANCE WITH AMERICAN NATIONAL STANDARD Z133.1-1972 AND ASPLUNDH MAINTENANCE MANUAL FOR AERIAL LIFT, MODELS LR 42 LR 45 LR 50. SPECIFIC ABATEMENT DOCUMENTATION IS REQUIRED
