Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employmennt and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to improperly installed and/or maintained cranes: IN THE FOLLOWING LOCATIONS, EMPLOYEES WERE EXPOSED TO THE HAZARD OF BEING STRUCK BY CRANE COMPONENTS AND/OR LOADS: A.RECEIVING DOCK: THE WIRE ROPE ON THE FIVE TON P&H UNDERHUNG CRANE WAS NOT SECURED TO THE DRUM AND FREE HANGING, CAUSING THE ROPE TO TANGLE ON THE DRUM. ALSO, THE CRANE HAD NOT BEEN SUBJECT TO A THOROUGH INSPECTION IN THE PAST YEAR AND WAS NOT SUBJECT TO REGULAR WIRE ROPE INSPECTIONS. B. FASTENER PLANT, ORANGE TEAM: THE WIRE ROPE ON THE TWO TON P&H UNDERHUNG CRANE WAS DAMAGED IN SEVERAL PLACES. THE WIRE ROPE WAS NOT SUBJECT TO REGULAR INSPECTIONS. ALSO, THE CRANE HAD MADE PHYSICAL CONTACT WITH THE 1000 NATIONAL DIE HOIST MOUNTED ON A PIVOTING BEAM AND NO PROVISIONS HAD BEEN MADE TO PREVENT SUCH AN OCCURANCE IN THE FUTURE. ONE FEASIBLE AND ACCEPTABLE METHOD TO CORRECT THE HAZARD, AMONG OTHERS, IS TO: A.FOLLOW THE MANUFACTURER'S GUIDELINES AND/OR ESTABLISHED INDUSTRY GUIDELINES REGARDING THE INSTALLATION OF HOISTS AND RELATED COMPONENTS. B.IMPLEMENT A PROGRAM OF REGULAR, DOCUMENTED INSPECTIONS OF HOISTS AND RELATED EQUIPMENT AND PERFORM MAINTENANCE AS INDICATED BY THE INSPECTIONS. SPECIFIC ABATEMENT DOCUMENTATION IS REQUIREDATION
