29 CFR 1910.119(d)(3)(i)(B): The employer did not ensure that process safety information pertaining to the equipment in the process was included in the piping and instrument diagrams (P&IDs). On or about October 2, 2015, and times prior to, at the MEMC Pasadena, Inc. facility in Pasadena, TX, when the employer failed to include on the Piping and Instrument Diagram valves utilized for isolation in the 92300 on page 02-20-92-24-FD-06B for the following piping : a) 1" SLN-7424-151 / 1/2" FX-7415-154 (K-92350 Compressor) b) 4" SLN-7439-154 / 4" SLN-4829-154 / 1 1/2" SLN-7422B-154 / 1 1/2" SLN-7422A-304.2 (D-92350 1st Stage Inlet Dampener to K-92350 Compressor) c) 1 1/2" SLN-7422-304.2 / 2" FX-7417-154 (K92350 Compressor to D92351 2nd Stage Discharge Dampener) d) 2" WCS-7141-125 (To E-92353 Interstage Cooler) e) 2" FX-7419-154 (D-92352 to flare)
