At the topsoil pile located at the Ricelli Northern Concrete Plant, on or about 10/03/15: A front end loader operator was not wearing a seatbelt while operating the vehicle. The operator fell from the moving loader, sustaining fatal injuries. Abatement documentation is required for this item.
