Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing hazards by not wearing seatbelts while operating forklifts: a) Cintas Corporation, Philadelphia, PA - Employees were exposed to death or serious physical harm in that the employer did not ensure that employees who operate the forklift truck wear the seatbelt, equipped with the YALE truck, model GlC030BFNUAV076. A fatality occurred on 4/27/99 when a fork truck tipped over, crushing the operator. NOTE: ABATEMENT DOCUMENTATION REQUIRED Among other methods, one feasible and acceptable abatement method to correct the hazard is to ensure the wearing of seatbelts while operating a forklift.
