Text For Citation: 01 Item/Group: 001 Hazard: LOCKOUT
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The %% employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of a rotating pipe: On or about February 19, 1992, employee(s) were exposed to a rotating 1 inch pipe that was being installed by a Ridgid 400 A pipe threading machine. No method was provided to effectively shut off power to the machine other than the motor control switches located nine feet away from the machine operator. Among other methods, feasible and acceptable abatement methods to correct this hazard would be to provide fast switches for all Ridgid 400 A pipe threading machines as recommended by the manufacturer in their publication #TF-535-74-R.
