29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition: a) On or about October 1, 2015- job site at 74 Onderdonk Ave, Ridgewood, NY 11385 the employer permitted employees to work in a facility that had refuse consisting of cardboard, plastic bags full of rags, plastic wrapping, old tires, and assorted other waste to accumulate at several locations through out the work site. b) On or about October 4, 2015- job site at 74 Onderdonk Ave, Ridgewood, NY 11385 the employer permitted employees to work in a facility with a restroom that floods with water during times of heavy rain. WRITTEN ABATEMENT DOCUMENTATION IS REQUIRED.
