29 CFR 1910.132(a): Protective equipment was not used when necessary when a hazard of process or environment capable of causing injury or impairment of the body through physical contact was encountered. On or about September 18, 2015, and at times prior thereto, two employees were changing air filter on the area on top of the clean room and were not provided with fall protection, exposing employees to a fall hazard of approximately 10 feet.
