29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: a)On or about October 26, 2011, and at times prior, Zavanna LLC, as controlling employer, did not ensure that Precision Drilling Company LP and Sidewinder Energy Consulting, Inc., subcontractors, protected their employees from fire/explosion hazards while working on a drill rig in that, employees were not wearing Flame Retardant Clothing.
