29 CFR 1910.23(a)(8)(ii): A floor hole into which persons can accidentally walk was not guarded by a floor hole cover of standard strength and construction: a) On or about October 25, 2011 and times prior, employees were exposed to injury in that, the cellar cover had a 2 foot by 2 foot opening. Helmerich & Payne International Drilling Co., dba H&P, Inc., was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 1910.23(a)(8)(ii) which was contained in OSHA inspection 310584354 citation number 1 item 1 issued on 01/26/2007, and became a finial order on 4/11/2007 with respect to a workplace located at TX HWY 186, Raymondville, TX 78580.
