29 CFR 1926.1051(a): A stairway or ladder was not provided at all personnel points of access where there is a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, and/or personnel hoist was provided: a) 268 Cold Spring Rd., West Springfield, MA: On or about September 25, 2015, an employee was climbing on the internal truss framing of the roof structure to reach the roof deck which was in excess of 10 feet above the walking surface.
