29 CFR 1910.24(h): Standard railings were not provided on the open sides of all exposed stairways and stair platforms: a) Stairway to the Second Floor Agitator Room - On or about September 23, 2015, employees, using the stairway to access the agitator room, were exposed to a fall hazard of 8 feet because the wall side of the stairway was not equipped with a standard railing as defined in 29 CFR 1910.23(e)(1). There was no mid-rail on the wall side of the stairway where the wall panel had been removed, exposing employees to a fall hazard of 8 feet into a flour hopper. No abatement certification or documentation required.
