29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees: a) On or about September 15, 2015, and at times prior thereto, the employer failed to ensure that the energized switch junction box near the flammable storage cabinet/chemical waste storage in the "GM" area was covered. This made the 120V live wiring vulnerable to damage and employees exposed to contact with live parts, which were capable of causing electrical shock and/or burns.
