Text For Citation: 01 Item/Group: 001 Hazard: UNAPEQUIP
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm in that employees were using a Cedar Rapids, Single Impeller, Impact Breaker that had a modified transmission/clutch lever. The modification was not approved by the manufacturer. Due to the positioning of the extension with regards to the control panel, an accidental engagement of power to the impeller was possible on or about June 3, 1997. One feasible and acceptable method of abatement, among others, to correct the hazard is to remove the pipe extension and return the machine to it's original,"as manufactured", configuration.
