Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm in that employee(s) were exposed to crushing injuries from shifting and/or falling load(s) or overturning forklifts: (a) Rear Yard Area Forklift operator on Hyster forklift # 65613 transferring two bundles of rebar twenty feet in length, weighing approximately 4170 pounds, from flat bed truck to storage location with forks near full extension, on or about 08/12/97. ONE FEASIBLE AND ACCEPTABLE METHOD OF ABATEMENT WOULD BE TO: (1) Follow 1969 Edition American National Standard Institute Standard B56.1-1969,Safety Standard for Powered Industrial Trucks Section 604 Traveling, Letter I which states "Travel with load engaging means or load low and, where possible, tilted back. Do not elevate the load except during stacking. (2) Follow 1993 Edition American National Standard Institute Standard B56.1-1993, Safety Standard for Powered Industrial Trucks Section 5.3 Traveling Item 5.3.10 which states "Travel with load engaging means or load low and, where possible, tilted back. Do not elevate the load except during stacking". (3) Follow the Hyster Operating Manual, part # 897413 Operating Procedures page 33 which states " Whether the lift truck is loaded or empty, do not travel with the load or carriage in a raised position" (4) Follow Scotty's Inc. Forklift Training Manual, Load Handling page 14 which states, " Never drive with the load up in the air, it could topple your forklift". (b) Rear Yard Area Forklift operator on Hyster forklift # 65616 was not wearing seatbelt while transferring two bundles of Rebar from truck flatbed to storage location, on or about 08/12/97. ONE FEASIBLE AND ACCEPTABLE METHOD OF ABATEMENT WOULD BE TO: (1) Follow 1993 Edition American National Institute Standard B56.1-1993, Safety Standard for Low Lift and High Lift Trucks Section 5.3 Traveling, Item 5.3.19 which states " An active operator protection device or system, when provided, shall be used. Operator protection in the event of tipover is intended to reduce the risk of entrapment of the head and torso between the truck and the ground but may not protect the operator against all possible injury" (2) Follow the Hyster Operating Manual, part # 897413 Operating Procedures page 27 which states " Warning, Before operating the lift truck FASTEN YOUR SEAT BELT". (3) Follow Scotty's Inc. Forklift Training Manual, Operating Techniques, page 9 which states, "If your forklift has a seatbelt, put it on and make sure its tightly clasped before starting the machine". ABATEMENT VERIFICATION REQUIRED FOR THIS ITEM.
