Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: the employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being pulled into and crushed by the rotating parts of a pipe threading machine: (a)Section 2 Ramp E, employees operated a Ridgid 300 pipe threading machine which was activated by a toggle switch that would not automatically shut the machine off if the operator was inadvertently caught up in its rotating parts; on or about 3/11/97. ABATEMENT NOTES: Among other methods, one feasible and acceptable method to eliminate this hazard is to follow the American National Institute Safety Standard for Portable Pipe Threading Machines and Portable Power Drives - Safety Requirements (ANSI B208.1-1982), Section 4.4, which states as follows: "a momentary contact device shall be used to control the power input to the cord- connected portable pipe threading machine or power drive. If a foot switch is used, it shall be guarded".
