Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to striking/crushing hazards: a). Assembly Area and throughout the entire facility to include jib and other cranes - The Underhung monorail Coffing 1/2 ton crane which had been moved to this area did not have its structure load tested; was missing mechanical stops for the trolley on the rail; no frequent (daily visual) inspections by the operators of the crane's chains, hooks, limit switches, trolley, swivels, and safety devices for physical defects; no records of the inspection of critical items; and no preventative maintenance records as required by the American National Standards Institute B30.16 - 1973, Monorail Systems and Underhung Cranes an B30.11 - 1973, Overhead Hoists, on or about 04/11/95. Among other methods, one feasible and acceptable method to correct this hazard is to train crane and hoist operators on when and how to conduct frequent inspections, immediately report unsafe conditions of the crane to management for action. Maintenance personnel also need to receive training on the requirements of inspection records, the need for load testing before use after installation, and the installation of rail stops for the hoist trolleys.
