29 CFR 1910.132(c): Personal protective equipment was not of safe design and construction for the work to be performed: Roof: On, or about, September 28, 2015, personal fall arrest equipment provided to the plant Electrician was not suitable for use in that: A. A suitable anchor point was not provided for the personal fall arrest equipment to be attached to. A vertical structural member was used that required the employee to pass out of the protective area of the roof mounted guardrail system and stand at the edge of roof to attach the lanyard. B. The retractable lanyard was attached to the employee's harness backwards, the retractable housing was attached to the rear D-ring, which was not in accordance with the manufacturer's instructions. C. The lanyard was wrapped around a vertical structural member and secured back onto itself using a carabiner, which is not in accordance with industry guidelines. D. The age of the harness was unknown, in that the information on the tag had worn off. The lanyard was approximately 16 years old. The equipment supplied exceeded the recommended life expectancy of personal fall arrest equipment in regular use.
