Section 5(a)(1) of the Occupational Safety & Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that, employees were exposed to: the hazard of delayed rescue and possible injury during entry into and exit from an electrical manhole on the west side of the switch gear building CP427/402 on October 18, 1993. Methods to abate include but not limited to: institute a confined space entry program meeting the requirements of OSHA 1910.146 and/or ANSI Z117.11989, the program should include: 1)Effective training of the authorized entrants, attendants and all affected employees. 2)Legible completion of all portions of the permit, the permit should specify the nature of the work to be completed and any necessary equipment, including rescue equipment. AND 3)Use of an effective retrieval device such as a Winch and Tripod with a fall arrest system where self rescue may fail and vertical retrieval is
