Text For Citation: 01 Item/Group: 001 Hazard: FALLING

Section 5(a)(1) of the Occupational Safety & Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that, employees were exposed to a serious fall hazard because the hoist wire rope, the safety lines, and the scaffold rigging were not installed to meet accepted rigging practices. A feasible and useful method to correct this hazard, among other methods, would be to isolate the wire rope from the electrical cable, and provide softeners at sharp edges where safety lines tie. Connect the tie-backs to the eyelet of the hoist cable. Install outrigger beams to reduce the stress on the hoisting equipment: Jobsite: The hoist wire rope, the safety lines, and the scaffold rigging were not installed on the Spider scaffold in a manner which met accepted rigging practices.
