29 CFR 1910.147(c)(1): Respondent did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energization, start up or release of stored energy could occur and cause injury, the machine or equipment was isolated from the energy source and rendered inoperative: (a) Fill Room 2, on or about 9/2/15: Respondent did not utilize lockout procedures when an employee cleared a jam on the Evergreen EQ-5 Filler 6. (b) Fill Room 2, on or about 9/2/15: Respondent did not train employees who performed activities including, but not limited to cleaning operations, clearing jams, changeovers, etc. on its filler machines as authorized employees to ensure that they acquired the knowledge and skills required for the safe application, usage and removal of energy control devices.
