Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The %% employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to uncontrolled release of energy in that the established guidelines for caustic feed were not followed due to, among others, inadequate training during production of Phenol-Formaldehyde resins. (a) Phenol-Formaldehyde (PF) resin production, area K-1 vessel - Established product manufacturing instruction (PMI) was not followed during caustic feed resin production due to inadequate training and inappropriate location of valves and gauges, on or about 2/28/92. Among other methods, one feasible and acceptable abatement method to correct this hazard is to follow the product manufacturing instruction, through adequate training for all personnel involved and to have accurate methods/means of controlling the batch(es).
