29 CFR 1910.22(c): Covers and/or guardrails were not provided to protect personnel from the hazards of open pits, tanks, vats, ditches, etc: a) On or about October 27, 2011, and at times prior, employees were exposed to slipping, falling, and tripping hazards while crossing a ditch that was not provided with a cover/guardrail (19 inches deep). b) On or about October 27, 2011, and at times prior, employees were exposed to falls of approximately 6 feet in that, the reserve pit was not provided with a fall protection system.
