Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: On or about May 6, 1993, during the erection of the bowling alley, employees were exposed to the hazards of serious injuries such as lacerations, broken bones, or death from a roof collapse while installing metal plate connected wood trusses approximately 87 foot long. Inadequate lateral bracing and inadequate top chord diagonal bracing were used for the truss erection. Among others, feasible and acceptable methods of abatement to include: Establish proper spacing of both lateral bracing and top chord diagonal bracing by use of a registered professional engineer for trusses over 60 foot in length as recommended by the Truss Plate Institute, and/or by utilization of truss manufacturer's recommendations during the erection of the trusses.
