Text For Citation: 01 Item/Group: 001 Hazard: FALLING

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a fall hazard from an elevating platform, locally modified. a.IN THE PLANT, BUILDING TR-1, J-BAY: On or about February 10, 1994, employees exposed to fall hazards while working on chiller/condenser units from three (3) manually propelled Ballymore cantilevered platforms which were modified and altered as follows: model #BL-C14SP, serial #2822 was missing guard rails on the right corner; model #BL-C14SP, serial #2821 was missing outriggers; model #BL-C14, serial #BL-2675 was missing toe board on front section. The modifications and alterations were made without being approved by the manufacturer as required in ANSI A92.3-1990, Section 6.13 (Manually Propelled Elevating Aerial Platforms). These modifications weakened the structural manufacturer.
