Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by the falling rig mast: Rig # 22; An Ideco Full-View Mast was raised by using the traveling block without using weight indicators to gauge the weight pull on the mast while an employee was allowed to climb up in the mast before the automatic locking cams locked the mast legs in position. The mast fell in the direction that it was being pulled. Among other methods one feasible and acceptable abatement to correct this hazard is to obtain and follow the Ideco Full-View Mast raising procedures provided by the manufacturer which requires using weight indicators and ensure that no employees are allowed to get up in the mast until the locking cams are locked and the brakes are locked.
