29 CFR 1910.138(a): The employer did not select and require employees to use appropriate hand protection when employees hands were exposed to hazardous conditions. On or about the 31st of July 2015, employees engaged in grinding operations were exposed to amputation hazards in that the employer did not select and require appropriate hand protection for grinding operations.
