29 CFR 1926.651(j)(1): Adequate protection was not provided to protect employees from loose rock or soil that could pose a hazard by falling or rolling from an excavation face: a) On or about October 7, 2011 and at times prior, Kelley Trucking Inc. did not ensure that employees were protected from falling rock hazards. Employees were instructed to begin work on a buttress wall without, one of the following measures being taken to address falling rock hazards. 1. The area was not scaled after major earth moving processes left loose rock on the excavation face. 2. The wire mesh used to contain falling rocks was not installed above the area where the buttress wall was to begin. 3. No protective barricades were installed on the face of the slope to prevent loose rock from entering the work area.
