29 CFR 1926.451(e)(5)(i): Ramps and walkways 6 feet (1.8 m) or more above lower levels did not have guardrail systems that comply with subpart M of this part -- Fall Protection: a) Speedwell Forge Lake Dam; Front Side of Concrete Wall Form - Employees using a walkway to access a form scaffold were not protected by a guard rail system and were exposed to fall hazards up to 9 feet - 1 inch to the ground below, on or about July 10, 2015. b) Speedwell Forge Lake Dam; Rear Side of Concrete Wall Form - Employees using a walkway to access a form scaffold were not protected by a guard rail system and were exposed to fall hazards up to 10 feet - 10 inches to the ground below, on or about July 10, 2015. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
