Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The hazard of being crushed by moving parts of conveyor system: a) SSM 4 & Bay 230: Interlocked access door needed. b) Access ladder, rear of cafe by pass through between Bulk and International. c) Next to bay 239B, Col 18B, International. d) International Col B21 and Bay 230. e) Col B28, Bay 225. f) Col 23C Elevator pump room. g) Col F27 access gate. e) Col F27 Military side, ladder to catwalk and access to cart rollers. f) Col J27 by RO13 access ladder with crossover. g) Col J26 access ladder with gate. e) Stairway by Col 25G. Employee(s) were exposed to a caught-in hazard due to the lack of electronically interlocked gates provided to shut down conveyor system when employee accesses the SSM4 area. Reference The American National Standards Institute. ANSI B20.1-1976, Section 6.16, item 6.16.2.2. One feasible and acceptable abatement method to correct this hazard is to install electrically interlocked gates at every entrance access to the conveyor. Violation observed on or about 10/12/94
