Section 5(a)(1) of the Occupational Safety and Health Act of 1970: %% The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the recognized hazard of inhalation of toxic Hydrogen Sulfide gas. The following condition contributed to the existence of this hazard: On or about March 10, 1992, at the Catalytic Polymerization (Cat Poly) unit there was no way to detect leaks when the Hydrogen Sulfide (H2S) gas was removed from the process feed in the caustic tower W95. Leakage of H2S gas in the work area may occur through gauge glasses, flanges, valves, and during the caustics change operation. Feasible abatement method(s) to correct this hazard may include, but not limited to: 1.) Provision of monitoring/alarm devices at the Cat Poly to detect and provide audible and/or visual warning of an accidental/uncontrolled release of Hydrogen Sulfide gases.
