29 CFR 1910.178(m)(1): Trucks were driven up to employees standing in front of a bench or other fixed object. On or about 1 July 2015, and at times prior thereto, employees operating a Hyster stand-up forklift drove it in such a manner as to expose both themselves and others to the hazard of being struck by the lift. 1) On July 1, 2015, an employee drove the lift in reverse while another employee was standing behind the lift next to a pallet rack. 2) On or about June 15, 2015, an employee crushed his foot between the lift and pallet rack when he jumped out of the operator's cab before the lift had stopped moving.
