29 CFR 1926.403(i)(2)(i): Except as required or permitted elsewhere in this subpart, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by cabinets or other forms of enclosures, or by any of the following means: In the bathroom for room 202, live parts of electrical equipment operating at 110 to 220 VAC were not guarded against accidental contact, as there was no cover on an electrical panel box 11 inch by 13 inch opening. The bottom of the panel box was about 64 inches from the concrete floor. Employees in the bathroom chipping off tile with a jackhammer were exposed to the hazard of contacting electricity.
