SECTION 5(A)(1) OF THE OCCUPATIONAL SAFETY AND HEALTH ACT OF 1970: THE EMPLOYER DID NOT FURNISH EMPLOYMENT AND A PLACE OF EMPLOYMENT WHICH WERE FREE FROM RECOGNIZED HAZARDS THAT WERE CAUSING OR LIKELY TO CAUSE DEATH OR SERIOUS PHYSICAL HARM TO EMPLOYEES IN THAT EMPLOYEES WERE EXPOSED TO: A) BEING STRUCK AND/OR CRUSHED BY FORGINGS, HOIST AND ASSOCIATED PARTS WHILE WORKING IN THE FORGE SHOP AT PRE-HEAT OVEN #2018, IN CLOSE PROXIMITY OF P&H HOIST #3556 AND IT'S SUSPENDED LOAD. THE HOIST HAD AN INOPERATIVE UPPER LIMIT SWITCH, LEAKING GEAR BOX, AND WIRE ROPE TWISTED/OR CROSSED ON WIRE ROPE DRUM, WHICH ARE NOT IN ACCORDANCE WITH ANSI B30.16-73. AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THES E HAZARDS IS TO PROVIDE A PROPERLY OPERATING HOIST AND/OR REPAIR EXISTING HOIST IN ACCORDANCE WITH MANUFACTURER'S RECOMMENDATIONS. AND PERFORM FREQUENT (DAILY TO MONTHLY) AND PERIODIC (MONTHLY TO ANNUAL) INSPECTIONS OF THE HOISTING EQUIPMENT TO DETERMINE UNSAFE CONDITIONS SUCH AS, BUT NOT LIMITED TO, UNSAFE CONDITIONS NOTED ABOVE. ALL DEFICIENCIES NOTED MUST BE RECORDED AND CORRECTIVE ACTION TAKEN. B) BEING STRUCK AND/OR CRUSHED BY FORGINGS, HOIST AND ASSOCIATED PARTS WHILE WORKING IN THE HOT INSPECTION, FORGE SHOP, BUILDING #6, IN CLOSE PROXIMITY OF P&H HOIST #5823 AND IT'S SUSPENDED LOAD. THE HOIST HAD A IMPROPERLY OPERATING UPPER LIMIT SWITCH, AND GEAR BOX LEAKED OIL, WHICH ARE NOT IN ACCORDANCE WITH ANSI B30.16-1973. ALL DEFICIENCIES NOTED MUST BE RECORDED AND CORRECTIVE ACTIO N TAKEN. C) BEING STRUCK AND/OR CRUSHED BY FORGINGS, HOIST AND ASSOCIATED PARTS WHILE WORKING IN BUILDING #6, FORGE GRIND, IN CLOSE PROXIMITY OF P&H HEVI- LIFT HOIST #4354 AND IT'S SUSPENDED LOAD. THE HOIST HAD A DAMAGED SHEAVE BLOCK GUARD AND GEAR BOX LEAKED OIL, WHICH ARE NOT IN ACCORDANCE WITH ANSI B30.16-73. AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THESE HAZARDS IS TO PROVIDE A PROPERLY OPERATING HOIST AND/OR REPAIR EXISTING HOIST IN ACCORDANCE WITH MANUFACTURER'S RECOMMENDA- TIONS, AND PERFORM FREQUENT (DAILY TO MONTHLY) AND PERIODIC (MONTHLY TO ANNUAL) INSPECTIONS OF THE HOISTING EQUIPMENT TO DETERMINE UNSAFE CONDITION S SUCH AS, BUT NOT LIMITED TO, UNSAFE CONDITIONS NOTED ABOVE. ALL DEFICIENC- IES NOTED MUST BE RECORDED AND CORRECTIVE ACTION TAKEN. d) BEING STRUCK AND/OR CRUSHED BY FORGINGS, HOIST AND ASSOCIATED PARTS WHILE WORKING IN BUILDING #6, FORGE GRIND, IN CLOSE PROXIMITY OF P&H HOIST #5801 AND IT'S SUSPENDED LOAD. THE HOIST HAD A BIRD CAGED WIRE ROPE, WHICH IS NOT IN ACCORDANCE WITH ANSI B30.16-1973. AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THESE HAZARDS IS TO PROVIDE A PROPERLY OPERATING HOIST AND/OR REPAIR EXISTING HOIST IN ACCORD- ANCE WITH MANUFACTURER'S RECOMMENDATIONS, AND PERFORM FREQUENT (DAILY TO MONTHLY) AND PERIODIC (MONTHLY TO ANNUAL) INSPECTIONS OF THE HOISTING EQUIPMENT TO DETERMINE UNSAFE CONDITIONS SUCH AS, BUT NOT LIMITED TO, UNSAF E CONDITIONS NOTED ABOVE. ALL DEFICIENCIES NOTED MUST BE RECORDED AND CORRECTIVE ACTION TAKEN. e) BEING STRUCK AND/OR CRUSHED BY DIES, HOIST AND ASSOCIATED PARTS WHILE WORKING IN DIE SHOP, IN CLOSE PROXIMITY OF 5 TON HOIST (CH-#3946-B) AND IT' S SUSPENDED LOAD. THE HOIST HAD DEFECTIVE BRAKES AND IMPROPERLY OPERATING CONTROL MECHANISMS ( ONCE THE FUNCTION CONTROL BUTTONS WERE PUSHED THE HOIS T HAD A LENTHY DELAY BEFORE RESPONDING TO COMMAND). AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THESE HAZARDS IS TO PROVIDE A PROPERLY OPERATING HOIST AND/OR REPAIR EXISTING HOIST IN ACCORDANCE WITH MANUFACTURER'S RECOMMENDATIONS, AND PERFORM FREQUENT (DAILY TO MONTHLY) AND PERIODIC (MONTHLY TO ANNUAL) INSPECTIONS OF THE HOISTING EQUIPMENT TO DETERMINE UNSAFE CONDITIONS SUCH AS, BUT NOT LIMITED TO, UNSAF E CONDITIONS NOTED ABOVE. ALL DEFICIENCIES NOTED MUST BE RECORDED AND CORRECTIVE ACTION TAKEN. f) BEING STRUCK BY AN OVERHEAD LOAD BY USING HOISTING ROPE THAT HAD SEVERAL BROKEN STRANDS IN IT AND HAD NOT BEEN REPLACED WHEN REPLACEMENT WAS REQUIRED. A FEASIBLE AND USEFUL METHOD TO CORRECT THIS HAZARD WOULD BE HAZARD WOULD BE TO VISUALLY INSPECT ROPES PRIOR TO EACH USE AND REPLACE THEM WHEN NEEDED. THE FOLLOWING ARE THE AREAS WHERE HOISTING ROPE WAS FOUND TO BE IN POOR CONDITION : 1) MACHINE SHOP-MACHINE #6001 : THE 1/4" WIRE ROPE ON HOIST WG #5512 WAS IN SERVICE AND HAD SEVERAL BROKEN WIRES ON IT. 2) TUMBLAST DEPT: THE 1/2" TON HOIST LOCATED AT TUMBLER #4967, HAD A WIRE ROPE THAT HAD SEVERAL BROKEN WIRES ON IT. g) THE HAZARDS ASSOCIATED WITH OVERLOADING A HOISTING MECHANISMS BY NOT HAVING THE RATED CAPACITIES OF EACH HOIST CLEARLY MARKED ON EACH HOIST. THE HOISTS IN THE FOLLOWING AREAS EITHER HAD NO LEDGIBLE MARKINGS ON THEM TO INDICATE THEIR RATED CAPACITIES OR HAD MARKINGS THAT DID NOT COINCIDE WITH THE TRUE RATED CAPACITIES OF THE HOISTS. 1) MACHINE SHOP: HOIST #5433, RATED AT 500 LBS, HAD A 1000 LB LABLE ON THE THE BLOCK OF THE HOIST. 2) MACHINE SHOP: HOIST #6002 HAD NO MARKINGS ON IT TO INDICATE ITS RATED CAPACITY. 3) MAINTENANCE MACHINE SHOP: HOIST # E4606 , RATED AT 500 LBS, HAD A 1000 LB LABLE ON THE BLOCK OF THE HOIST. 4) MAINTENANCE MACHINE SHOP: P&H HOISTS # 6123 HAD NO MARKINGS ON IT TO INDICATE ITS RATED CAPACITY. 5) MAINTENANCE MACHINE SHOP: HOIST # 2381 HAD NO MARKINGS ON IT TO INDICATE ITS RATED CAPACITY. A FEASIBLE AND USEFUL MRTHOD TO CORRECT THIS HAZARD WOULD BE TO OBTAIN THE RATED CAPACITIES OF EACH HOIST FROM THE MANUFACTURER AND MARK THIS RATING ON THE HOIST SO THAT IT IS LEGIBLE FROM THE GROUND. h) BEING STRUCK BY AN OVERHEAD LOAD WHEN OVERHEAD HOISTS WERE USED TO LIFT MATERIAL THAT WEIGHED MORE THAN THE RATED CAPACITY OF THE HOIST. OVERHEAD HOISTS LOCATED AT THE OPERATORS STATIONS OF VERTICAL TURRET LATHES, WERE USED TO LIFT PARTS THAT WEIGHED IN EXCESS OF THE RATED LOAD OF THE HOIST. TWO SPECIFIC INSTANCES OF OVERLOADING OCCUR- RED ON THE FOLLOWING HOISTS: 1) HOIST # 3691 (1/4 TON)- PART #69680 (OPERATION 790) WEIGHED 741 LBS PRIOR TO MACHINING (685 LBS AFTER MACHINING) AND WAS LIFTED BY THIS 500 LB HOIST. 2) MACHINE # 5560 (1/2 TON HOIST) - PARTS WEIGHING 1008 LBS (PART + SLINGS AND HOOKS) WERE BEING LIFTED BY THIS 1000 LB HOIST. A FEASIBLE AND USEFUL METHOD TO CORRECT THIS HAZARD WOULD BE TO INSURE THAT EACH OPERATOR KNOWS THE WEIGHT OF THE MATERIAL THAT IS BEING HANDLED AND INSURE THAT NO MATERIAL WEIGHING IN EXCESS OF THE RATED LOAD OF THE HOIST IS LIFTED. i) BEING STRUCK AND/OR CRUSHED BY FORGINGS, HOIST AND ASSOCIATED PARTS WHILE WORKING IN THE HOT INSPECTION, FORGE SHOP, BUILDING #6, IN CLOSE PROXIMITY OF P&H HOIST #5823 AND IT'S SUSPENDED LOAD.
