OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm in that employees were exposed to struck by hazards created by sudden pressure buildup inside the vacuum system of the Federal Signal Vactor Guzzler XCR truck: a) PGW Port Richmond, Philadelphia, PA: On June 23, 2015, while spent Molsiv LNG-5 was being vacuumed from Vessel C-160A, two metal caps located on the silencer of the Federal Signal Vactor Guzzler XCR Vacuum Truck were ejected as the result of an unexpected accumulation of hydrocarbons in the hopper that caused a combustion event. Among other methods, one feasible and acceptable means to abate these conditions is to ensure the vessel is properly purged pursuant to the Molsiv manufacturer's guidance. abatement method to correct these hazards is to follow the truck manufacturers vacuuming procedures.
