Text For Citation: 01 Item/Group: 001 Hazard: FALLING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: Upon receipt employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of falling from the elevated platform of the Cougar Electric/hydraulic order picker serial number 9-04841068, as the safety floor lock designed to prevent movement of the machine when the work platform was elevated was inoperative. Additionally, the safety interlock mechanism designed to prevent the work platform from being elevated without the floor lock engaged, had been deliberately defeated on 7/30/90/. Among other methods, one feasible and acceptable abatement method to correct this hazard is to repair the safety floor lock and interlock so that the work platform could not be elevated unless the floor lock was properly engaged against the floor to stabilize the equipment.
