Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of becoming entangled in the exposed rotating part which could result in fractures, crushing injuries and/or death: a)Fourth floor - Employees using a "RIGID 300" pipe threading machine, serial #720194, which was equipped with a toggle type ON/OFF switch, which would not automatically shut off if the operator became caught up in the rotating parts of the machine. Among other methods, one feasible and acceptable method to reduce this hazard is to establish and enforce safe operating procedures, such as those stipulated in the American National Standard for pipe tools-cord connected portable pipe threading machines and portable power drives safety requirements. (ANSI B208.1-1982). elements essential to an acceptable safe operating procedure include at a minimum (1) Installation of a properly guarded foot switch. The foot switch would automatically shut power off to the pipe threader when the operators foot is removed from the foot switch for any reason. (2) Provide training and education to all operators on the appropriate procedures for the safe use and operation of this type of equipment.
