29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: The employer is failing to adequately train employees to recognize, avoid, and prevent struck by hazards associated with deep drilling while working around cranes with suspended loads. The most recent occurrence was on or about June 24, 2015 at the job site located at 18th and P Streets Lincoln, NE where evidence of inadequate training included, but is not limited to: i. Two employees that were not essential to the tilt-up operation continued to work in the fall zone as they failed to recognize the struck-by hazard and feasible methods to avoid and mitigate the risk during tilt-up operations. ii. Inexperienced employees were given insufficient instruction on the means and methods to work safely and around suspended loads that included rebar cages and single sections of rebar.
