Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being crushed while operating a Cushman Turf Truckster: (a) Location - Jobsite, 96 Main Street, Wenham, MA: On 6/25/04, an employee was exposed to and received serious injuries while operating the Cushman Turf Master, 3-wheeled maintenance vehicle when it tipped over. Among other methods feasible and acceptable to correct this hazard includes, but are not limited to: a. Follow the instruction provided in the operator's manual. b. Retrain employees in the hazards associated with the operation of this type machine. c. Develop and implement a standard operating procedure for filling sand traps, approaching them from the lower most point.
