Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a. Injuries including but not limited to amputation of hands and limbs from being caught between the hoist way wall and the elevator car. One feasible abatement method is to install doors or gates at each entrance of the elevator car as required by the ANSI standard for existing elevator: ANSI 17.3-1993 3. 4.2. This condition was observed on 12/23/92.
