Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY
Section 5 (a) (1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to rolling hazards: (a) On august 20, 1991 at approximately 12:30 p.m. or sometime thereof, an employee was struck and killed by a rootwad rolling down the mountain because the employer was not alert at all times for logs, trees, rootwads, rocks or other objects that could roll toward him, and because a general inspection was not conducted of the area to be worked that day to identify logs, rootwads, rocks, chunks or other objects that may roll toward him in accordance with Alaska State Logging standard 07.115(P)(2) and 07.115(P)(1). (b) Among other methods , one feasible and acceptable method to correct this hazard is to conduct and document training to new employees on hazards of the job task, hazards of the work area, personal protective equipment and procedures pertaining to the job, and emphasize that employee should stay alert. Additionally, ensure all objects are identified and removed from the area prior to routine work activitiesbegining to prevent recurrence of the same type accident in the future. This should be supervised.
