Text For Citation: 01 Item/Group: 001 Hazard: LIFTING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: the hazard of being struck-by and/or crushed-by loads of steel plate and other materials or equipment which were attached to crane hoist(s) by the following "below-the-hook lifting devices", i.e., lifting beams/spreader bars, which neither had the rated load capacity visibly displayed, nor an attached nameplate, or other permanent marking, which displayed the manufacturer's name and address, the serial number, lifter weight, and rated load: (Reference ANSI/ASME B30.20-1993, Below-The-Hook Lifting Devices, Section 20-1.2.1(a) and (b)). (a)North Structural Department, Bay #2 - The 22 foot long yellow spreader bar made of stiffened pipe, plus square tubing and alloy steel chains, used to move long sections of steel plate, and (b)North Structural Department, Bay #3 - The 14 foot long by 5.125 inch wide by 8 inch tall yellow spreader bar made of welded back-to-bach channel beams and stiffener plates, plus three (3) sets of two (2) chain-attached "C" hooks, used to move steel plate, and (c)North Structural Department, Bay #4 - The 14 foot long by 5.125 inch wide by 8 inch tall yellow spreader bar made of welded back-to-bach channel beams and stiffener plates, plus three (3) sets of two (2) chain-attached "C" hooks, used to move steel plate, and (d)Sheet Metal Department, West of Panel Line #2 - The 40 X 40 inch lifting device made of 2 inch metal tubing and used for changing out sheet metal mill dies, etc. (e)Sheet Metal Department - The "Special I-Beam Supported Cradling Device" for moving roofing panels, and (f)North Structural Department, Bay #4 - The shorter yellow spreader bar made of channel welded back to back, with short hooks at each end, used for miscellaneous lifting applications. Among other methods one feasible and acceptable method to correct this hazard is to visibly mark each under-the-hook lifting device with its appropriate rated load, and affix a data plate with identifying information to each lifting device.
