Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by the load should the overhead hoist, supporting structure, or below-the-hook lifting devices fail: a)In the workplace, at locations and equipment identified, on dates listed, the following overhead hoists had missing and/or damaged hook latches, unrated supporting structures, unlabeled controls, damaged controls, or damaged wire rope, including, but not limited to: 1.Aluminum Foundry, SWO# 2477, Cleveland Tramrail electrolift electric wire rope hoist: The electric control lines entering/exiting the pendant control had insulation removed exposing bare wires. Observed on or about 02-23-99. 2.Iron Foundry, SWO# 2545, pneumatic chain hoist: The controls were not labeled and there was no load rating on the hoist or the supporting structure. Observed on or about 02-22-99. 3.Coremaking Area, SWO# 2547, Centurion pneumatic chain hoist: There was no load rating on the supporting structure. Among other methods several feasible and acceptable methods of abatement to correct each of these hazards are to: 1.Determine load rating from structural design material. 2.Determine loading rating from structural engineering study. 3.Label the proper rating on each of the supporting structures. 4.Establish a log of each hoist for frequent and periodic inspection. 5.Label the controls for the direction of operation. 6.Repair/ replace controls. 7.Repair/ replace hook latches. 8.Follow requirements as listed in ASME/ANSI B30.16. 9. Follow requirements as listed in ANSI B30.11. 10.Replace wire ropes with defects such as severe kinking, etc. b)In the workplace, at locations and equipment identified, on dates as indicated, the following below-the-hook lifting devices had no rated load marking and no initial, frequent or periodic inspections performed, including, but not limited to: 1.Coremaking Area: A Morse lifting device, Serial# 172852, suspended by SWO# 2547 was not provided with frequent and periodic inspections. Observed on or about 02-22-99. 2.Shalco Area: Two lifting devices attached to Gardner Denver Hoists, were not provided with frequent and periodic inspections. Observed on or about 05-18-99. 3.Iron Foundry: Lifting device suspended by SWO# 2504 was not provided with frequent and periodic inspections and was not provided with any markings or load ratings. Observed on or about 02-22-99. 4.Iron Foundry: Lifting device suspended by SWO# 2486, was not provided with frequent and periodic inspections. Observed on or about 02-22-99. 5.Iron Foundry: Lifting Device suspended by SWO# 2489, was not provided with any markings or load ratings, and was not provided with any frequent or periodic inspections. Among other methods, several feasible and acceptable abatement methods to correct each of these hazards are to: 1.Prior to initial use, all new, modified or repaired lifting devices shall be inspected by a designated person and an operational test performed to verify compliance. 2.Mark the rated load capacity of the lifting device on the main structure or equivalent. 3.Conduct frequent and periodic inspections of the lifting devices for wear, deterioration or malfunction in accordance with ASME B30.20.
