Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by the load should the overhead hoist, supporting structure, or below-the-hook lifting devices fail: a)In the workplace, at locations and equipment identified, on dates listed, the following overhead hoists had missing and/or damaged hook latches, unrated supporting structures, unlabeled controls, damaged controls, or damaged wire rope, including, but not limited to: 1.Aluminum Foundry, SWO# 2477, Cleveland Tramrail electrolift electric wire rope hoist: The electric control lines entering/exiting the pendant control had insulation removed exposing bare wires. Observed on or about 02-23-99. 2.Iron Foundry, SWO# 2545, pneumatic chain hoist: The controls were not labeled and there was no load rating on the hoist or the supporting structure. Observed on or about 02-22-99. 3.Coremaking Area, SWO# 2547, Centurion pneumatic chain hoist: There was no load rating on the supporting structure. Observed on or about 02-22-99. 4.Coremaking Area, SWO# 2460, pneumatic chain hoist: There was no load rating on the supporting structure. Observed on or about 02-22- 99. 5.Shalco Area, SWO# 2491, hoist: There was no load rating on the supporting structure. Observed on or about 02-22-99. 6.Shalco Area: Two Gardner-Denver Hoists were supported by one supporting structure that did not have any load rating. Observed on or about 02-22-99. 7.Iron Foundry, SWO# 2532, electric wire rope hoist: There was no load rating on the hoist or the supporting structure. Observed on or about 02-22-99. 8.Iron Foundry, SWO# 1404, electric wire rope hoist: There was no load rating on the supporting structure. Observed on or about 02-22- 99. 9.Iron Foundry: SWO#'s 2487, 2528, and 2504 were all attached to the same supporting structure that did not have any load rating. Observed on or about 02-22-99. 10. Iron Foundry, floor molding area: An electric chain hoist had no load rating on the structure, no directional labels on the controls, and no hook latch. Observed on or about 02-22-99. 11.Iron Foundry, SWO# 2486, pneumatic chain hoist: There was no load rating on the supporting structure. Observed on or about 02-22-99. 12.Iron Foundry, SWO# 2489, pneumatic chain hoist: There was no load rating on the supporting structure. Observed on or about 02-22-99. 13.Foundry Maintenance, welding area, SWO# 2093, Yale electric wire rope hoist: There was no load rating on the supporting structure and the wire rope was overlapped onto itself on the drum. Observed on or about 02-22-99. 14.Foundry maintenance, pneumatic chain hoist: There was no load rating on the support structure. Observed on or about 02-22-99. 15. Iron Foundry, SWO# 2531, electric wire rope hoist: There were no load rating on the supporting structure or the hoist. Observed on or about 02-22-99. 16.Iron Foundry: SWO#'s 2531, 2537, 2539, 2538, electric wire rope hoists, were on the same supporting structure (SWO# 2404), which did not have any load rating. Observed on or about 02-22-99. 17.Aluminum Foundry, Hoist #12, pneumatic chain hoist: There was no load rating on the supporting structure and the hook latch was broken. Observed on or about 02-23-99. 18.Aluminum Foundry, SWO# 2465, electric wire rope hoist: The controls were not labeled and there was no load rating on the supporting structure. Observed on or about 02-23-99. 19.Aluminum Foundry, SWO# 2466, Cleveland tramrail, electric chain hoist: The controls were not labeled. Observed on or about 02-23-99. 20.Aluminum Foundry, SWO# 2464, hoist: The controls were not labeled. Observed on or about 02-23-99. 21.Aluminum Foundry: SWO#'s 2466, 2465, 2464, 2467, were all on the same supporting structure which was not labeled with a load rating. Observed on or about 02-23-99. 22.Aluminum Foundry, SWO# 2469, electric wire rope hoist: Controls were not labeled, no load rating for the supporting structure. Observed on or about 02-23-99. 23.Aluminum Foundry, SWO# 2468, electric wire rope hoist: Controls were not labeled, no load rating for the supporting structure. Observed on or about 02-23-99. 24.Building 33: The CM Lodestar electric chain hoist in front of the powder paint area was attached to a support structure that was not provided with a load rating. Observed on or about 02-24-99. 25.Plastics Department, SWO# 2083, Yale pneumatic chain hoist: The controls were not labeled, and the supporting structure was not labeled with the load ratings. Observed on or about 02-24-99. 26.Bin Wash, SWO# 2078, Yale pneumatic chain hoist: The controls were not labeled, there was no load rating for the supporting structure, and the hook latch was missing. Observed on or about 02- 24-99. 27.Power Plant, SWO# 2482, electric chain hoist: There was no load rating for the supporting structure, and there were no labels on the controls. Observed on or about 02-24-99. 28.Truck Maintenance, SWO# 2074, electric chain hoist: There was no load rating on the supporting structure. Observed on or about 02-24- 99. 29.CNC, SWO# 1984, pneumatic chain hoist: The controls were not labeled and there was no load rating for the supporting structure. Observedon or about 02-25-99. 30.Plating Department, pneumatic chain hoist: There was no load rating on the supporting structure. Observed on or about 2-26-99. 31.Chip Room, SWO# 2499, electric chain hoist: There was no load rating on the supporting structure. Observed on ore about 02-25-99. 32.Building# 2, SWO# 2054, pneumatic chain hoist: There was no load rating for the supporting structure. Observed on or about 02-25-99. 33.Building# 2, SWO# 2011, pneumatic chain hoist: There was no labeling of the controls, and there was no load rating on the supporting structure. Observed on or about 02-26-99. 46.CA1 Assembly, SWO# 2032, pneumatic chain hoist: There was no load rating on the supporting structure. Observed on or about 02-26- 99. 47.CNC Maintenance, SWO# 2558, pneumatic chain hoist: There was no load rating on the supporting structure. Observed on or about 02-26- 99. 48.Tank Wash Area, SWO# 2049, pneumatic chain hoist: The support structure was not labeled, and the support structure was in disrepair (connecting pins drifted). Observed on or about 02-26-99. 49.Tank Wash Area, SWO# 2047, pneumatic chain hoist: Support beams were not labeled, and supporting structure was in disrepair (connecting pins drifted). Observed on or about 02-26-99. 50.Tank Wash Area, SWO# 2490, pneumatic wire rope hoist: Controls were not labeled, and there was no load rating on the supporting structure. Observed on or about 02-26-99. 51.Enamel, SWO# 2089, pneumatic chain hoist: There was no load rating on the supporting structure, and controls were not legible (painted over). Observed on or abbout 02-26-99. 52. Building# 2 conveyor from CNC, SWO# 2040, Yale pneumatic chain hoist: There was no load rating on the supporting structure. Observed on or about 02-26-99. 53.Building# 2 Tool Room, SWO# 2059, CM electric chain hoist: There was no load rating on the supporting structure, and the control reel does not extend to the full length of the rail. Observed on or about 02- 26-99. 54.Building# 2, SWO# 2062, electric chain hoist: There was no load rating on the supporting structure. Observed on or about 02-26-99. 55.Building# 50 Tool & Die Repair, SWO# 2120, electric chain hoist: There was no load rating on the supporting structure, and the pendant control was missing the button for the down control. Observed on or about 03-03-99. 56.Building# 50 Tube Mill Process, SWO# 2565, Cleveland Tramrail electric chain hoist: There was no load rating on the supporting structure. Observed on or about 03-03-99. 57.Building# 50, SWO# 2087, electric wire rope hoist: There was no load rating on the supporting structure, hook latch was missing, and wire clips on hoist cable were attached backwards. Observed on or about 03-03-99. 58.Sheet Metal Shop, SWO# 2107, electric wire rope hoist: Load cable is damaged (kinked), load cable is crossed/overlapped on the drum, and the hook latch is damaged. Observed on or about 05-18-99. Abatement certification must be submitted for this item. Among other methods several feasible and acceptable methods of abatement to correct each of these hazards are to: 1.Determine load rating from structural design material. 2.Determine loading rating from structural engineering study. 3.Label the proper rating on each of the supporting structures. 4.Establish a log of each hoist for frequent and periodic inspection. 5.Label the controls for the direction of operation. 6.Repair/ replace controls. 7.Repair/ replace hook latches. 8.Follow requirements as listed in ASME/ANSI B30.16. 9. Follow requirements as listed in ANSI B30.11. 10.Replace wire ropes with defects such as severe kinking, etc. b)In the workplace, at locations and equipment identified, on dates as indicated, the following below-the-hook lifting devices had no rated load marking and no initial, frequent or periodic inspections performed, including, but not limited to: 1.Coremaking Area: A Morse lifting device, Serial# 172852, suspended by SWO# 2547 was not provided with frequent and periodic inspections. Observed on or about 02-22-99. 2.Shalco Area: Two lifting devices attached to Gardner Denver Hoists, were not provided with frequent and periodic inspections. Observed on or about 05-18-99. 3.Iron Foundry: Lifting device suspended by SWO# 2504 was not provided with frequent and periodic inspections and was not provided with any markings or load ratings. Observed on or about 02-22-99. 4.Iron Foundry: Lifting device suspended by SWO# 2486, was not provided with frequent and periodic inspections. Observed on or about 02-22-99. 5.Iron Foundry: Lifting Device suspended by SWO# 2489, was not provided with any markings or load ratings, and was not provided with any frequent or periodic inspections. Observed on or about 02- 22-99. 6.Aluminum Foundry: Lifting device suspended by Hoist# 12 was not provided with any markings or load ratings and was not provided with frequent and periodic inspections. Observed on or about 02-23-99. 7.Aluminum Foundry: Lifting device suspended by SWO# 2466 was not provided with any markings or load ratings, and was not provided with any frequent and periodic inspections. Observed on or about 02- 23-99. 8.Aluminum Foundry: Lifting device suspended by SWO# 2464 was not provided with any markings or load ratings, and was not provided with any frequent and periodic inspections. Observed on or about 02- 23-99. 9.Plastics Department: Two Barrel lifting devices that are suspended from SWO# 2083 were not provided with any markings or load ratings, and were not provided with any frequent or periodic inspections. Observed on or about 02-24-99. 10.Building# 4: Lifting device suspended by SWO# 2096 was not provided with any markings or load ratings, and was not provided with any frequent or periodic inspections. Observed on or about 02- 26-99. 11.Sheet Metal Department: Lifting device suspended by SWO# 2018 was not provided with any markings or load ratings, and was not provided with any frequent or periodic inspections. Observed on or about 05-18-99. 12.Tank Wash Area: Lifting device suspended by SWO# 2049 was not provided with any markings or load ratings, and was not provided with any frequent or periodic inspections. Observed on or about 02- 26-99. 13.Tank Wash Area: Lifting device suspended by SWO# 2047 was not provided with any markings or load ratings, and was not provided with any frequent or periodic inspections. Observed on or about 02- 26-99. 14.Tank Wash Area: Lifting device suspended by SWO# 2490 was not provided with any frequent or periodic inspections. Observed on or about 02-26-99. 15.Enamel Area: Lifting device suspended by SWO# 2089 was not provided with any markings or load ratings, and was not provided with any frequent or periodic inspections. Observed on or about 03-03-99. 20.Electric Melt: Magnetic lifting device suspended by SWO# 1205 was not provided with any cautionary labeling, markings or load ratings, and was not provided with any frequent inspections. Observed on or about 03-04-99. 21.Electric Melt: Magnetic lifting device suspended by SWO# 1206 was not provided with any cautionary labeling, markings or load ratings, and was not provided with any frequent inspections. Observedon or about 03-04-99. 22.Bin Wash: Lifting device supported by SWO# 2078 was not provided with any markings or load ratings, and was not provided with any frequent or periodic inspections. Observed on or about 02-24-99. 23.Building# 50, Tool & Die Repair: Lifting device used under various SWO#'s was not provided with any markings or load ratings, and was not provided with any frequent or periodic inspections. Observed on or about 03-03-99. Abatement certification must be submitted for this item. Among other methods, several feasible and acceptable abatement methods to correct each of these hazards are to: 1.Prior to initial use, all new, modified or repaired lifting devices shall be inspected by a designated person and an operational test performed to verify compliance. 2.Mark the rated load capacity of the lifting device on the main structure or equivalent. 3.Conduct frequent and periodic inspections of the lifting devices for wear, deterioration or malfunction in accordance with ASME B30.20.
